After stepping up to the plate to challenge his cousin, Jacob Fatu is set to face off with Roman Reigns for the World Heavyweight Championship at WWE Backlash 2026.
This main event showdown should offer some intriguing surprises depending on which direction the outcome turns out to be.
Whether it be stars debuting and returning or a shocking result, this title bout between Reigns and Fatu can have a great impact on the landscape of WWE moving forward.
Here are six potential surprises for Roman Reigns vs. Jacob Fatu at WWE Backlash 2026.
6. Solo Sikoa & The MFTs Interfere To Help Roman Reigns

Solo Sikoa debuted as Roman Reigns’ enforcer at WWE Clash at the Castle 2022
On the April 24 edition of Friday Night SmackDown, Jacob Fatu defeated Solo Sikoa in the main event.
Post match, The MFTs jumped Fatu, but “The Samoan Werewolf” returned the fury and assaulted every member of the group with a steel chair before driving Tonga Loa through the commentary table.
With Fatu now officially set to challenge Roman Reigns for the World Heavyweight Championship, it may be Sikoa and The MFTs coming to get their revenge at the most inopportune time for their former stablemate.
The MFTs interfering to cost Fatu the World Heavyweight Championship would be fitting and bring all of the elements of the OG Bloodline together in this story at WWE Backlash 2026.
5. Zilla Fatu Debuts
While The Bloodline has several members who are signed to WWE, there are also those part of the Samoan dynasty family that are outside of the company.
Over the past few years, Zilla Fatu has been making a name for himself across the independent scene.
Fatu has also made it clear that he would like to one day sign with WWE and join the rest of The Bloodline.
Joining forces with Jacob Fatu or acknowledging “The OTC,” Zilla making his debut at WWE Backlash 2026 could open the door for different stories to be told.
4. Jacob Fatu Acknowledges Roman Reigns After Loss
On the April 20 WWE Raw After WrestleMania 42, Jacob Fatu stepped up to Roman Reigns to challenge for the World Heavyweight Championship at Backlash 2026.
After hearing this challenge, Reigns told Fatu that he will have to acknowledge “The Tribal Chief” if he comes up short against him.
While Fatu will be a tough test for Reigns, it feels like “The OTC” will not be losing the top prize on Monday Night Raw so soon after winning the gold at WrestleMania 42.
Reigns defeating Fatu could lead to “The Samoan Werewolf” acknowledge his “Tribal Chief” after the loss at the WWE Backlash 2026 premium live event and join forces with the Bloodline moving forward.
3. Jacob Fatu Defeats Roman Reigns Clean
At WWE WrestleMania 42 Night Two, Roman Reigns defeated CM Punk to win the World Heavyweight Championship.
In what will be his first challenge for this reign, Reigns will now defend the title against Jacob Fatu at WWE Backlash 2026.
Considering he just won the gold three weeks earlier, it would be quite surprising to see Reigns lose the title at the very next PLE.
Fatu defeating Reigns clean feels like the most unlikely ending to the main event of Backlash 2026, but it could be the one that would leave all the fans stunned to see in Tampa, Florida.
2. The Rock Returns To Be Special Guest Referee

The Rock and Roman Reigns teamed together to take on Cody Rhodes and Seth Rollins at WWE WrestleMania 40
When he made his return to WWE in 2024, The Rock joined forces with Roman Reigns and The Bloodline.
Despite their interactions since then including The Rock giving Roman Reigns the ula fala following his win over Solo Sikoa in Tribal Combat, it has seemed like the two cousins have had different agendas over the past two years.
Now that The Bloodline is seemingly coming back together, “The Final Boss” could reinsert himself into this family business at WWE Backlash 2026.
Coming to determine who will lead the family moving forward, The Rock may name himself the special guest referee for the Roman Reigns vs. Jacob Fatu main event at WWE Backlash 2026.
1. The Usos Turn On Roman Reigns To Join Forces With Jacob Fatu
Following Roman Reigns’ win over CM Punk at WWE WrestleMania 42, The Usos joined forces with “The OTC” once again.
However, Jacob Fatu has shown just what he is capable of right before Jimmy and Jey’s eyes.
Knowing just how dangerous he can be, The Usos warned Reigns that Fatu is a different kind of beast coming after his World Heavyweight Championship.
Turning against the man who manipulated them for years, The Usos could choose to unite with Fatu against Reigns, helping him to win the title at the WWE Backlash 2026 premium live event in Tampa.
